Health questionnaire from the 'West of England Insurance Company', tear at top, from Dorchester with printed address panel to 'Mr. Lewis, Secretary, West of England Insurance Office, Exeter.' dated 12th of November 1831 and charged '9'. On front fine to very fine strike of the boxed 'No.5' receiving house hand stamp of Maiden Newton and on reverse good strike (over flap) of the 'Dorchester/P.y Post' hand stamp in black (DT247).
